PHS PROCEDURES

Services of the school nurse are available to students throughout the day. Students who are ill should report to class and ask their teacher for a pass to go to the nurse's office. In those cases where the student is too ill to remain in school, the parents and/or guardians will be notified. Under no circumstances should an injured or ill student go home without permission. Anyone who must take medication during the day should report to the nurse.

Ill or injured students should only leave the school campus at the discretion of the professional school nurse after an appropriate assessment of the illness or injury. it is important that the nurse be able to communicate with the parent regarding the student's needs.

All students enrolled in high school are required by law to have an up-to-date immunization record on file in the Nurse's Office.
