    Sports Medicine Physical Information 



    Welcome to Liberty Athletics


    Liberty High School believes that athletics is an integral part of the educational experience and offers an excellent athletics program. Our program includes 15 varsity sports for both boys and girls. Through healthy competition we hope to instill life-long character traits including teamwork, responsibility and work ethic. 



    Athletic Fees


    The Student Participation Fee is $100. There is a family maximum per year of $200. This fee is paid once a year by all athletes, band members and spirit line members and covers all sports for the school year. This fee is due at the beginning of the first sport the student participates in each school year. 



    Sporting Event Admission Fees


    $5.00 for Adults
    $3.00 for Students (10 and over with ID)
    Student Pass $50.00 for the year
    Family Pass  $150.00 for the year
    Student and family passes are good at all games in the Peoria School District, excluding Playoff games and tournaments. 



    Athletic Clearance
     for 2019-2020


    Incoming 8th graders and those new to Liberty: Please visit our Athletic Clearance page for more information and directions on how to create an account for your student and complete their clearance.  




    Beginning March 1st you can start the Athletic Clearance Process for the 2019-2020 school year. Please make sure to use the drop-down date at the top of your athletes account and change it to the 2019-2020 school year when you want to update/upload for next year. 


    Verify all information on the registration page is up to date including address, phone number, insurance, etc.


    Choose every sport you think your athlete might be interested in so they are cleared for it in advance. If they don't participate in that sport it will not affect them.


    Any physical from 3/1/19 forward will cover your athlete through 5/31/20. Make sure to use the AIA physical forms. They can be downloaded from "Register My Athlete" or from the Liberty website under "Athletic Clearance". There must be a stamp from the medical office where you had your physical in order for the physical to be valid. If there is no stamp the physical will be rejected. 


    All physicals for the 2018-2019 school year, regardless of the date it took place, expire on May 31, 2019. 


    Click on "New Registration" at the top of the page and follow the prompts.


    Make sure all the boxes are checked and the e-signatures are complete and checked at the bottom. 


    Upload new physical forms at any time. 


    If the brainbook test was never completed, the athlete will need to complete the test and the certificate will need to be uploaded. If it says "Complete" in green next to Brainbook then nothing else needs to be done for that. This only needs to be done one time and is good for all four years. 


    This update process should not take more than 5 minutes. 


    **Students that are participating in summer camps will need to have this process completed and their Physical and Brainbook uploaded before they can participate.
