2024 Sports Camps

Directions:

1. All Participants - Print and complete the camp registration form from below
2. High School Participants: Print and complete this Release of Liability Form
3. Youth Participants (4th-8th grade for the 24-25 SY): Print and complete this Youth Camp Registration form
4. Return completed registration form and Release of Liability or Youth Camp Registration form with payment to our Centennial HS Bookstore.

Incoming freshman student athletes need to be registered in akivate/Register My Athlete,  a physical completed by a physician and uploaded into RMA for approval, before they will be able to participate in any of the summer camps. Bookstore Information:

• During the school year: Open Mon, Tue, Thurs, Fri  7am-3pm  Wed.  7am-1pm

• During the summer:  Open Mon & Wed.  12:00pm-4:00 pm  Tue. & Thurs.  7:30am-11:30 am - *Closed Fridays during the summer
• Forms of payment : Debit/Credit Cards, Check or Money Order *cardholder must be present
• Coaches DO NOT accept payment
